This program is open to any Women of Today chapter that has already donated $100 to the USWT Founders Charities; please submit by May 15.

By signing, I agree that I have read and understand the conditions and the guidelines for this grant, and that I will comply with the following:

* Funds awarded will be spent on the project described in the Chapter Grant application.

* Grantee will be held liable for funds, if not spent as approved.

* The Foundation will not be held liable for any claims or project losses.

* Grantee will account for expenditure of Foundation funds separately from regular project funds.

* Within thirty (30) days of project completion, the grantee will forward two (2) copies of the final report, including actual budget, indicating how all funds were used, what the project accomplished, and an evaluation of how the project was managed. The final report is due to the Foundation President.

* Grantee will return all unused Foundation funds.
