IMPORTANT REMINDER REGARDING HEARTWORM TESTING FOR PUPPIES AND ADULT DOGS:

* Dogs adopted out who tested negative and puppies should be tested 6 months after adoption.

* Dogs adopted out who were treated for heartworm should also be tested 9 months after they have completed their treatment.

ALL dogs/puppies must be on year round heartworm prevention for life.

Adopter will pay a $200 spay/neuter fee, in addition to the non-refundable adoption fee, which MAY be returned to the Adopter provided that all terms and conditions outlined in this agreement are met.

Adopter agrees to have the dog spayed/neutered and to submit proof of spay/neuter to Save A Lab Rescue no later than 12 months from the date of adoption. Documentation may be faxed to 336-464-2126 or emailed to info@savealabrescue.org.

Adopter may choose to have the surgery performed by one of Save A Lab Rescue's approved vets, in which case Save A Lab Rescue will apply the spay/neuter fee towards the cost of the surgery. Save A Lab Rescue will pay the vet directly and will cover the cost of the spay/neuter surgery only; any additional vetting (microchip, bloodwork, etc.) will be paid for by the Adopter. Adopter may also choose to have the surgery performed by their personal vet, in which case Save A Lab Rescue will refund the spay/neuter fee to the Adopter provided that the documentation is received no later than 6 months from the adoption date.

In the event that the dog suffers from a life-threatening condition that does not allow this agreement to be honored, the Adopter must provide a statement from their veterinarian describing the health condition and the reason that it precludes the dog from having the surgery. The statement must also establish the earliest date that the procedure can be performed, and the Adopter will schedule the surgery to be performed at that time. All other terms of this spay/neuter agreement remain in effect.

If Adopter fails to meet this requirement, the spay/neuter fee will be forfeited, but the Adopter will still be legally obligated to comply with the terms of this agreement.

Adopter understands that permanent ownership of this dog is contingent upon execution of this agreement, and will not be transferred to the Adopter until the dog has been spayed/neutered. Failure to comply with this spay/neuter agreement will be considered a breach of the adoption contract. In the event that this agreement is violated, the Adopter agrees to return the dog back to Save A Lab Rescue with no refund. The Adopter will also be responsible for any legal fees incurred by Save A Lab Rescue associated with reclamation of the dog.
