Lights of Remembrance 2025
Transitions GriefCare presents Lights of Remembrance, an opportunity to honor our loved ones and acknowledge our strength as a community of grievers. Transitions invites you to attend on ONE of the following dates:
Wednesday, January 8 from 5:30 - 7:30pm
-OR-
Thursday, January 9 from 5:30–7:30pm
Fill out the form below to request a luminary and submit a photograph in honor of your loved one. For best results, send a photo in portrait orientation. Orders must be received by Wednesday, December 11th to ensure adequate time for printing.
Note: Your loved one’s name, photo, and tribute message will be viewable to all those who attend this event.
