Please fill out the following application if you are wanting to surrender a pet to Perfect Paws Animal Rescue.

Although we do our best to accommodate every unwanted pet, we are not able to accept all pets into our program. We are a foster based rescue that has a committee that carefully reviews all surrender requests and works to find a suitable foster home for each pet.

Due to an extremely high volume of surrender requests for unwanted pets, foster homes are not always available. There may be an extensive wait to place your pet in a foster home.

IF YOU ARE ABLE TO FOSTER YOUR PET DURING THE SEARCH FOR A NEW HOME YOUR APPLICATION WILL HAVE A HIGHER CHANCE OF APPROVAL. THIS WILL PROVIDE YOUR UNWANTED PET A BETTER OPPORTUNITY TO BE PLACED IN A LOVING FOREVER HOME MORE QUICKLY AND WILL HELP TO REDUCE THE STRESS ON THE PET FROM HAVING TO MOVE INTO A TEMPORARY FOSTER HOME.

Please answer all questions on the form honestly and provide us with as much information on the unwanted pet as possible. Missing information will only cause further delay in the processing of your request.

If your pet is selected to be taken into our program someone will contact you to discuss it further. If you not heard anything us in 3 months please feel free to submit a new application.
