The Awards Committee selects winners using an objective points-based system based on nominee’s activity in the past 18 months. Awards will not be given to candidates who do not meet the minimum criteria threshold. This form must be completed in its entirety by midnight on

to be considered eligible for a Leadership Award. No late questionnaires will be accepted.
