2025 APPLICATION FORM

Information regarding our 2025 application process:
FEES:
Session fees are:

$85.00 per 5 hour session and
$101.75 per 6 hour session

We are also offering optional extended care in 2025.

Our Extended care sessions will operate from 2pm -5pm on Monday and Wednesday and from 3pm -5pm on Tuesday and Thursday. This will extend your child's session to a total of 8 hours at a total cost of $136.00 per day.

If you would like to add extended care to your child's day, simply include this option in the Extended care section below.

(Please note that if you are not looking to commence from the first available date in 2025, and would like to commence at a later date please complete our WAITING LIST Form instead. Please also note that this Application Form is for a Term/ongoing booking. If you are looking for a casual booking only, please contact us at info@playcentre.org.au)

Up to 5 days of care per week can be provided per child.

Our Sessions operate from
9.00am - 2.00pm - Monday, Wednesday and Friday
9.00am - 3.00pm - Tuesday and Thursday .

Our terms run in accordance with Victorian school terms and we are closed during all school holiday periods.

All fees are due in advance for the term ahead prior to commencement of any sessions.

FEES POLICY

APPLICATION PROCESS: Please complete the following application form without delay in order to secure a place on your preferred day/s. You will be notified as soon as possible of acceptance based on availability.

IMMUNISATION: Please note that all children must be fully immunised for their age. Proof of Immunisation from the Australian Immunisation Register will be required to confirm enrolment.
APPLICATION FEE - TERMS AND CONDITIONS

$203.50 each 6 hour session selected.

is payable when submitting an application for your child for 2025.

Please note that an additional deposit of $50 per day is applicable for all sessions of extended care selected.

• This application fee will be deducted from your term fee invoice

• THIS APPLICATION FEE IS NOT REFUNDABLE, unless we are unable to offer you a place on day or days requested.

• Applications will be accepted, and enrolments made, in order of date form AND payment are received.

PLEASE NOTE: No enrolment will be processed until the application fee has been paid.
