MNWT State President Filing
The following information should be completed by the candidate and submitted no sooner than December 1. All required items should be submitted to the chairman of the Elections Committee as soon as possible (no later than 12:00pm Friday at Annual Convention). No one is officially a candidate until this form is approved.
Use the File Upload field to include two (2) additional pages of information using one (1) side of the page only. The first of these should be prepared by the president of your local chapter in letter form declaring chapter support of your candidacy and explaining how your chapter can support you as officer. The second should be a statement of belief in this organization and why you wish to hold this office. (Information from these two pages will be used for newspaper or NEWSLET purposes.) In addition, please upload an electronic photo of the candidate suitable for media use.
Signatures can be done by your mouse, or your finger on a writable screen.
Responses to the online form will be viewed by the state chairman of the board, who is also the chairman of the Elections Committee. Fill in as much information as you know. Upon completion of the form, click Submit; you will receive a confirmation screen if your form has been successfully submitted. (rev 2015)
