I understand and agree that my bail bondmen, shall have control and jurisdiction of me during the term for which the bond is executed and that you have the right to surrender me on this bond at any time that you desire, as provided by law. I further agree and understand that I will not leave the jurisdiction of the court without the permission of the bondsmen; nor commit any further offenses that will subject me to any subsequent arrest, by any authority nor fail to pay any premium due; and I will notify the my bondsmen promptly of any change of address and carefully comply with the specific instructions given to me by the bondsman.

For good and valuable consideration, the undersigned co-signor hereby agrees to indemnify and hold harmless the bail bondsman/ agent, as surety from any loss that they may sustain as a consequence and incident to the execution of the foregoing bond and the said co-signor here, and acknowledges that if the connection with the execution of the foregoing bond, that they are responsible and admits herein to their indebtness the said surety.

If you have a credit balance, the co-signor and the defendant agree to pay the bail bondmen the balance of the bond amount. If you fail to pay the balance due, a warrant will be issued for the co-signor and the defendant for THEFT OF SERVICES (O.C.G.A. 16-8-5).

Further, I understand and agree that failure to comply with any of the herein contained conditions shall be cause for my immediate surrender without any liability for the return of any premium. (Persons on Federal Bonds have other restrictions)

To bring the person back to the bonding company if he is in forfeiture. If you refuse to bring the person in for surrender, you may be charged with Federal Statue: Harboring a Fugitive: Title 18, 1701. Concealing person from arrest. Who ever harbors or conceals any person for arrest or warrant or process has been issued under the provisions of any law of the United States, so as to prevent discovery and arrest, after notice or knowledge of the fact that a warrant or process has been issued for apprehension of such person, shall be fined more than $1,000 or imprisoned not more that one year or both.

Co-signor will be required to pay a bounty hunter fee to locate the principal/ defendant and surrender them back to jail. Co-signor will be required to pay the bond in full plus any court costs and / or attorney fees within 30 days of the date of forfeiture. Notice is also given that if anyone gives false information about the defendant or themselves as a co-signers, they will be prosecuted for perjury in Superior court for falsifying a legal document. Also be sure to get a receipt for all money paid.

The Note is being executed by the undersigned to secure the bail bondsmen, upon forfeiture or entreasure of the surety bond posted on behalf of the co-signor/ indemnitor: FOR VALUE RECEIVED, the undersigned jointly and severally promise the bail bondsmen, surety, on demand, to pay to the order of the bail bondsmen the sum of the service fee plus all bail recovery fees and applicable court costs, or such other place or places as the owner or holder hereof may from time to time designate with interest at the rate of twelve percent per annum from call date until fully paid.

The maker and endorser of this note further agree to waive demand, notice of nonpayment and protest and in case suit shall be brought for the collection hereof or the same has to be collected upon the demand of an attorney, to pay all costs of collections, including, but not limited to, a reasonable attorney fee. Further, the defendant and indemnitor/ co-signor(s), respectfully waive all rights of exemption under the United States Constitution and Federal Laws, and the laws of the State of Georgia and any other state.

It is further agreed and specifically understood that this NOTE shall become NULL AND VOID in the event the said defendant appears in the proper court at the time and times so directed by the judges of competent jurisdiction until all obligations under the bond or bond(s) posted on behalf of the defendant have been fulfilled and the Surety is discharged of all liability, otherwise remains in full force and effect.

All parties of this note waive presentment, notice of non-payment, protests and notice of protest, and agree to remain fully bound notwithstanding the release party, extension or modification of terms, or discharge or any collateral for this NOTE. This note shall take effect as a sealed instrument and all monies are made due within ten days of the Defendants failure to appear writing of this promissory note, to be enforced in accordance with the laws of the payee's state or any state.

The declarant/ co-signor of the address listed has known the principal /defendant, presently incarcerated in the county jail, for a period of time as identified. I have provided all known names the defendant is known by in this form. False information shall result in the indemnitor/ co-signor arrest.

If the bounty hunter is unsuccessful in finding the defendant, the co-signor is totally responsible for the payment of the full amount of the bond plus all legal cost involved.

The co-signor must pay the bail recovery fee if found locally. If the defendant is not local or in a different state, the fee is twice the local rate plus all travel expenses. If a bail jumping warrant is filed and obtained, all fees and associated court cost are assumed by the co-signor.

Defendant agrees to pay the forfeiture amount of the sum owed plus any cost that the surety company incurs if the defendant fails to appear in court and a forfeiture of judgement is issued. If a bail jumping warrant is filed and obtained there is a administrative fee as well as a fee for the warrant which incur interest.
