Your Agreement With Miller Travel Company
  • Before begin making arrangements for your flight, hotel, car rental, tour, cruise, or other trip, we require that you sign this form; your “Signing” below will signify your agreement with the following terms and conditions:

    Miller Travel Company, LLC. (“Miller Travel Company”) acts as a sales agent for any airline, hotel, car-rental company, tour operator, cruise line, or other service provider named in your itinerary (“Suppliers”). Miller Travel Company is not responsible for acts or omissions of the Suppliers or their failure to provide services or adhere to their own schedules.

    Miller Travel Company assumes no responsibility for and shall not be liable for any refund, personal injury, property damage, or other loss, accident, delay, inconvenience, or irregularity which may be caused by: (1) any defaults, wrongful or negligent acts, or omissions of the Suppliers; (2) any defect in or failure of any vehicle, craft, equipment, or instrumentality owned, operated, or otherwise used or provided by the Suppliers; (3) any wrongful or negligent acts or omissions on the part of any other party not under Miller Travel Company’s control; and (4) any Supplier price drop after your travel arrangements have been confirmed, paid in full, or ticketed. You hereby release Miller Travel Company from all claims arising out of any problem covered in this paragraph.
  • I acknowledge that I am aware of any risks and alerts that have been issued to the country I am traveling to. It is my responsibility to address any concerns or review any updates as necessary. Miller Travel Company has no special knowledge regarding the financial condition of the Suppliers, unsafe conditions, health hazards, weather hazards, or climate extremes at locations to which you may travel. For information concerning possible dangers at destinations, Miller Travel Company recommends contacting the Travel Warnings Section of the U.S. State Department at (202) 647-5225 or www.travel.state.gov For medical information, Miller Travel Company recommends contacting the Centers for Disease Control at (877) FYI-TRIP or www.cdc.gov/travel

    I will assume full and complete responsibility for checking and verifying any and all passport, visa, vaccination, or other entry requirements of your destination(s), and all conditions regarding health, safety, security, political stability, and labor or civil unrest at such destination(s). You hereby release Miller Travel Company from all claims arising out of any problem covered in this paragraph. You agree that the courts in will be the exclusive jurisdiction for all claims brought by you or Miller Travel Company, and you hereby submit to the personal jurisdiction of those courts.

    For your protection, we strongly recommend that you purchase trip cancellation and travel accident insurance. However, no representation or description of the insurance made by our staff constitutes a binding assurance or promise about the insurance.

    We also strongly recommend that you use a credit card for your purchase, so that you can exercise your rights under the Fair Credit Billing Act if you do not receive the services you purchased.


  • Children under the age of 18 not traveling with both natural parents require a notarized letter signed by the absent parent(s) giving permission to the traveling parent or authorized adult to take the child out of the United States. This letter must contain the travel dates and the name and address of where the child will be staying.
  • The United States Government requires a valid passport for re-entry into the United States regardless of the departure date from any other country. Traveling to any country other than the United States of America regardless of age we are aware that we must possess a legal Passport, Visas and/or legal documents that may be required by that country. We understand that the passport must be valid for at least six months after our return date of travel. Please visit www.travel.state.gov for specific rules.

    I understand that passports of everyone traveling in my party must be valid 6 for at least 6 months after our return travel date.
  • It is our sole responsibility to have the proper legal documents for travel in our possession at all times during travel and in no way will we hold Miller Travel Company or it's agents be liable for misplacement or destruction of our official documents for travel. Names must match the required government issued ID exactly.
  • I will be responsible for all cancellation penalties or loss of monies that may incur for our failure to produce the proper legal documentation.
  • I understand that by booking this trip, I voluntarily assume all risks, along with my co-travelers, regarding exposure to Covid-19. I understand that there may be airline schedule changes, updated transportation, hotel and resort protocol and possibility of resort closures. Due to the pandemic I understand you may experience increased customs and immigration screening, which may include health questionnaires and temperature checks. It is my responsibility to check for updated health advisories, as this could change at the last minute before or during travel. Miller Travel Company, and its agents, is absolved of all liability related to Covid-19.
  • It is my responsibility to check for the latest and current state and government and employer travel restrictions before booking and before your trip, which may include self-quarantine for an amount of time after returning to your home. Travel insurance is encouraged in case of last minute changes or mandates.
  • I understand that it is my responsibility to check with the airline for any updates to schedule changes 24-48 hours prior to departure. This can be checked online while checking in for your flight as well.* Your travel agent will advise you if the airline has implemented schedule changes in advance if they have been notified.
