Welcome! This form is the first step in requesting a Japanese Hair Straightening appointment for new or returning clients (26+ weeks) since your last visit. It should take 5-7 minutes to complete.

Japanese Hair Straightening Consultation Purpose:
The purpose of this consultation is to ensure your Japanese Hair Straightening service is as successful as possible. We will assess your hair’s length, density, current condition, texture, and any previous chemical treatments to prevent any adverse reactions to the service.

This Online Consultation form allows us to understand your specific goals and requirements.

NOTE: All Japanese Hair Straightening services are available Wednesdays - Saturdays.

HOW TO RESERVE A JAPANESE HAIR STRAIGHTENING APPOINTMENT

STEP 1: Complete and submit this consult form.

STEP 2. Ms. CeCe (Stylist) reviews your form and provides personalized recommendations based on your hair's condition and your requirements. Additional information may be requested if necessary.

STEP 3: Email & Text Response
You will receive a response via email within 72 business hours. Joy, our appointment coordinator, checks messages from Tuesday to Saturday, 8 AM to 4 PM. You will receive a Confirmed Appointment Silk Press Quote & Summary about your hair's suitability and personalized recommendations.

STEP 4: Review and Confirm
Review your personalized quote and confirm available dates via email or text to our Client Service Coordinator.

STEP 5: Secure Your Japanese Hair Straighteing Booking

Submit payment for your Japanese Hair Straighteing service or utilize one of our payment plan options to secure your booking.

    HOW TO MEASURE YOUR HAIRS' LENGTH
    To accurately measure your hairs’ length, use FLEXIBLE MEASURING TAPE.

    Place the measuring tape at back of your head below the cranial bone. Secure the measuring tape with a bobby pin or clip. Stretch your hair alongside the length of the tape in order to find your hairs’ length inches or centimeters.

    Please select the correct length as pricing will be adjusted based on actual Measurements at the time of service.

    We are pleased to announce that we can permanently straighten slightly highlighted or balayage hair using a gentler formula. Please take note of the following important details:

    Mandatory Add-On Service:

    A Hair Rebuilding Treatment is required for all clients with slightly highlighted or balayage hair.

    Benefits of the Hair Rebuilding Treatment:

    1. Use your cell phone, as professional camera images may be too large to upload.
    2. For best results, ask someone to take the photos for you.
    3. Take the photos indoors in natural lighting (avoid flashes and direct sunlight).
    4. Submit recent photos that clearly show your true hair length and texture.

    Uploading accurate photos helps us assess your hair's current condition and provide an accurate estimate for pricing and service time.

    Please refrain from submitting photos of your hair in braids, ponytails, wigs, or similar styles. We need to see your natural hair length, density, and condition for evaluation. Any photos that do not clearly showcase these will not receive a quote or estimate, wasting both your time and the stylist's. Thank you for your understanding!

    Please note that full payment or a payment plan is required to secure a non-refundable and non-transferable reservation for Japanese Hair Straightening (JHST).

    Extensive Services: For services estimated to last over 4 hours, a payment plan or full payment is required to secure your reservation.

    We offer various payment options for your Japanese Hair Straightening (JHST) service, as detailed below.

    Our payment plan can cover both the service cost and home maintenance products, allowing you to pay at a rate that suits you through our third-party vendors.
  • ***IMPORTANT*** Once a Payment is submitted, please TEXT a screenshot of the confirmation page, so that we can fully confirm your reservation.

    Payment plan approvals must be finalized at least seven (7) days before your scheduled appointment to ensure your registration and reservation for the requested time slot.

  • JHST CANCELLATION POLICY (strictly enforced):


    We require at least 48 hours' notice for all cancellations prior to your appointment.

    Cancellations made less than 48 hours in advance will incur a 50% charge of the full-service price. If you cancel within 24 hours or no-show, your deposit will be forfeited, and a new deposit will be needed.

    Clients are responsible for canceling or rescheduling at least 48 hours in advance. We provide automated text and email reminders two days before your appointment.

    You may reschedule once at no charge if you give at least 48 hours' notice.

  • SPECIALTY APPOINTMENT LATE ARRIVALS POLICY:

    We provide a 15-minute grace period for clients who may be running late.

    If you arrive more than 15 minutes late, a late fee of $25 - $45 will be added to the cost of your service in order to begin the JHST service.

  • CHEMICAL ESTIMATES TERMS

    Please note that we can only provide an estimated total for all Chemical and Specialty services. The final cost may vary based on several factors, including:

    - Hair length and density (thickness)
    - Additional processing time needed
    - Required product usage
    - Any add-on services requested

    Some chemical services may also require extra applications of color, perm, relaxer, keratin, or straightening solutions.

    We aim to provide accurate pricing based on our experience. If the estimate exceeds the final cost, any overages can be applied to future services or products.

  • JAPANESE HAIR STRAIGHTENING POST 72-HOUR MAINTENANCE

    The JHST service requires relatively low maintenance after the mandatory 72-hour setting period.

    Do not wash your hair for 72 hours following the in-salon service.
    If your hair is exposed to moisture, please blow-dry it on medium heat and flat iron that section at 380 - 390 degrees until the 72-hour wait period is complete.

    After the stylist reviews your JHST consultation and the photos, we will send you an estimate/quote via email. This will allow us to address any questions or concerns you may have in a more personal manner.

    I understand that all color and chemicals are not created equal. Certain and non-professional hair color and chemicals can react in unexplainable ways. Therefore, a Shampoo & Knot Pre-Test will be performed the Day of Service.
    I understand that if the Shampoo & Knot Test reveals my hair is not healthy for this service, the stylist will NOT proceed.
    I understand that a PRE-SHAMPOO & KNOT TEST is recommended 7-14 days before my JHST service.

    I understand length pricing is based on the hair being elongated and can changed based on the stylist assessment at the time of service.

    I understand that there is an upcharge for Extra Bowls of Solution need to complete the service.
    I will not hold my stylist or Endless Creations Salon accountable for any weakening or damage that occurs during this process if I fail to disclose the above fore mentioned services have not been disclosed.
    I understand that by signing this agreement that all Salon Services and deposits are non-refundable.
    I understand that if I fail to follow proper care and instruction, including professionally recommended products to care for the integrity of my hair after a chemical service, I will not hold Endless Creations Salon accountable.
