I attest I am the copyright holder of the submitted material and no copyrights have been violated in its production.

Further, I hereby agree to indemnify and hold harmless Vh2 Networks and its employees, staff and agents, its successors and assignees from and against all losses, damages, injuries, claims, demands and expenses, including legal expenses whatsoever nature, arising out of the cablecasting of this program.

I authorize Vh2 Networks, to use the following personal information:

(1) My picture – including photographic, motion picture, and electronic (video) images.

(2) My voice – including sound and video recordings.

I hereby Grant to VH2 Networks, its subsidiaries, licensees, successors and assigns, the right to use, publish and produce, for all purposes, my name, picture of me in film or electronic (video) form, sound and video recordings of my voice, and printed and electronic copy of the information described in Section 1 and 2 above in any and all media including, without limitations, cable and broadcast television and the internet, and for exhibition, distribution, advertising, sales, press conferences, meeting, hearing, educational conferences, in brochures and other printed media. This permission extends to all languages, media, formats in marketing now known or hereafter devised. This permission is to continue forever unless agreed upon by both parties in writing.

I further grant VH2 all rights, titles, in interest that I may have in all finished pictures, negatives, reproductions and copies of the original print, and further grant VH2 the right to give, sell, transfer, any exhibits the printed copies or facsimile thereof, for marketing, communication, or advertising purposes, as it deems fit. I hereby waive the right to receive any payment from signing this release and waive the right to receive any payments from VH2 use of any of the material described above for any other purposes authorized by this release. I also wave any rights to inspect or approve finished photographs, audio, video, media, or advertising recordings and copy or print it matters or computer-generated scanned images and other electronic media that may be used in conjunction therewith or to approve the eventual use that it might be applied. I have read the foregoing and I fully understand the content.
