Chapter Reactivation Loan Application
The Minnesota Women of Today Foundation may issue an interest-free loan up to the amount of one hundred fifty dollars ($150.00) to a reactivated chapter who receives approval from the MNWT Foundation's Board of Directors, subject to the recommendation of the Chapter Management Vice President of the Minnesota Women of Today.
NOTE: this form requires two (2) signatures: one from the Chapter President, and Chapter Treasurer. Be sure to have all signatory representatives present when completing this form. Signatures can be done by your mouse, or your finger on a writable screen.
Responses to the online form will be viewed by the State Extensions Director, and the MNWT Foundation President by due date. Upon completion of the form, click Submit; you will receive a confirmation screen if your form has been successfully submitted. (rev 2015)
