ATTENTION PAYPAL CUSTOMERS:

Paypal will charge +4% of your order. Should you choose Paypal as your method of payment we will send an invoice to your provided email on this form. Your order will not be printed until payment is received. Thank you.

Customers choosing cash payment require a 50% deposit prior to printing. Cash payments must be made at our store location. Rush orders are only accepted before 12pm NOON Monday through Friday.
