PURPOSE OF THE CONSULTATION
This online form helps us understand your hair goals and schedule the right service for you. It's the first step in requesting a Virgin or Retouch Relaxer appointment at Endless Creations Salon.

Note: All Chemical Relaxer/Texturizer services are available Wednesdays - Saturdays.

HOW TO RESERVE A CHEMICAL RELAXER/TEXTURIZER APPOINTMENT

STEP 1: Complete and submit this First Visit Chemical Relaxer/Texturizer consult form.

STEP 2. Ms. CeCe (Stylist) reviews your form and provides personalized recommendations based on your hair's condition. Additional information may be requested if necessary.

STEP 3: Email & Text Response
You will receive a response via email within 72 business hours. Our appointment coordinator, checks messages from Tuesday to Saturday, 8 AM to 4 PM. You will receive a Confirmed Appointment Chemical Relaxer/Texturizer Quote & Summary about your hair's suitability and personalized recommendations.

STEP 4: Review and Confirm
Review your personalized quote and confirm available dates via email or text to our Client Service Coordinator.

STEP 5: Secure Your Chemical Relaxer/Texturizer Booking

Submit your Preferred method of payment for your requested Chemical Relaxer/Texturizer service or utilize one of our payment plan options to secure your booking.

IMPORTANT NOTES:
- You may be placed on our waitlist, with an approximate wait time of 1-4 weeks.

- You have 72 hours (3 days) to respond to our Confirmed appointment Silk Press Quote & Summary email. If we do not receive a response, an appointment will not be scheduled.

- We will take non-identifiable photos and/or videos for social media to highlight our Chemical Relaxer/Texturizer services.

    Hair length impacts how much relaxer is required and the total processing time - longer hair may require more product and care
  • WHAT LENGTH DOES YOUR HAIR NEED TO BE FOR A RELAXER/TEXTURIZER HAIR SERVICE?

    Your hair should be at least 2+ inches long

    HOW TO MEASURE YOUR HAIR LENGTH
    For an accurate measurement, use a flexible measuring tape. Start at your scalp and extend the tape along the length of your hair to determine its length in inches or centimeters.

    Hair density affects how much cream relaxer is required for proper saturation and processing - more density may require more product and service time.
  • FOLLOW THE INSTRUCTIONS BELOW FOR AN ACCURATE READING
    - Place your hair into a ponytail using a ponytail holder (wrapping 2x's).
    - Wrap a flexible tape measure around the base of your ponytail (this reveals your hairs' density).

    HD - High Density Hair requires more time and product usage.

    This helps us to QUOTE CORRECT pricing and allocate and anticipate the amount of time and product usage your GinaCurl service may be.

    If you are suffering from major breakage or damage, we recommend 2 Options.

    1. Damaged Hair Repair Treatment - $130 per session - (3-5) sessions recommended to bring hair back to life before applying a chemical to your hair. This is an In-Salon Treatment Brings your hair Back to Life. This treatment will restore the missing elements back into the hair to a healthy balance.

    2. Mizani Butter Blend Relaxer Service - Pricing Varies: Based on Retouch or Virgin Application.

    Designed to gently smooth rough, brittle and frizzy hair. This is a sensitive scalp relaxer that transforms texture in healthy way. This hair straightening treatment was made for fine, color-treated hair.

    Offers Texture Versatility: Hair is Straightened but offers a Smooth Texturized Relaxed look. Offer 70-90% Curl Reduction

  • NOTEICE: BRAID REMOVAL & DETANGLING.

    If you are just removing braids, twist, extensions, etc. after an extended period of time, please Shampoo, Condition and Detangle your hair 4 DAYS before your Relaxer appointment and oil your scalp.

    The excess time it takes to detangle matted hair is NOT included in our service times. Should you arrive with matted, tangled hair, we reserve the right to cancel your appointment.

    Clients are NOT permitted to detangle their hair or remove braids, twist, extensions at their scheduled appointment. Hair Detangling is a separate service and fees will apply.

  • POROSITY & ELASTICITY TEST

    A Porosity & Elasticity Test is required THE DAY of your Relaxer service at NO additional cost.

    The Porosity & Elasticity determine the following:

    - The hairs' health
    - Formula Strength to use
    - Formula processing time.

    If the test reveals the hair is not healthy enough we WILL NOT proceed with the Relaxer service and all remaining balances are due.

    We will perform a Porosity & Elasticity Test 2-7 Days to assess and pre-screen your hair before your Relaxer service in order to determine the best course of action to take.

    This is a Porosity & Elasticity Test Only appointment scheduled for up to 30 minutes. This is a separate service.

    Your clothes may be exposed to the chemicals and products despite our best efforts to protect them. We advise that you dress down at your Relaxer appointment

    There is a risk of your jewelry (earrings, necklaces, etc.) being damaged or lost during your service(s). We advise that your remove them before your service(s).

  • Genetics, medications, hormones and your overall health. All of this plays a role in making up your chemical make up, including your hair. What may work for one person, may not work well for another due to the differenes in how your body reacts.
    Previous Chemical services - professional or at home: Whether lightened or darkened by a box or professional, or permed or relaxed, all of these services present their own risk. A faded Hair color remains in the hair until it is cut off.
    Well Water, hard water, and copper pipes - heat reactions #1: Mineral deposits from water and pipes can build up in your hair and cause a reaction with the active ingredient: Ammonium Thioglycolate (A.K.A - THIO). The reactions can be UNPREDICABLE
  • I acknowledge that this is a virtual hair analysis and consultation with a Licensed Certified Hair Stylist
    I have provided all information regarding previous treatments to my hair and/or color that may or may not affect the outcome of my service
    I understand Failure to provide true or accurate information can result in possible damage to my hair as well as lack of treatment consistency or effectiveness.
    I understand that My stylist will explain to me the process they recommend for my hair type, texture, condition and desired results.
    I understand that Relaxer Treatments are designed to permanently straighten over-curly hair textures by using a chemical relaxer product.
    I understand that my stylist will give me aftercare instructions and product recommendations to best care for, preserve and prolong my Relaxer treatment results.
    I am not pregnant or nursing, and I have advised my stylist of any potential concerning medical conditions including asthma or respiratory disease

    I understand that my stylist has been trained in the service(s) technique and that they will do their best to create the best results
    I release my stylist, Endless Creations Salon and any affiliates from any liability during or after my Relaxer treatment service if the process has unexpected or undesired results.
    I agree to all aftercare instructions and understand that my Relaxer service is non-refundable and non-negotiable
    In order to book my Relaxer treatment I agree to pay a non-refundable retainer to secure my appointment slot. The deposit will be applied to the balance due.
