I authorize Life's a Journey Vacations to charge the credit card indicated in this authorization form according to the terms outlined below. This payment authorization is for the goods/services described, for the amount indicated only.

I certify that I am an authorized user of this credit card and that I will not dispute the payment with my credit card company; so long as the transaction corresponds to the terms indicated in this form.

I have reviewed all trip details and confirm that all information is correct and any alterations to the trip may result in extra fees. I acknowledge that I have received and agree to the terms and conditions of the reservation.

I acknowledge that I am purchasing this trip using a travel agent and not with the airline/hotel directly therefore the transaction cannot be disputed due to flight/hotel service dissatisfaction.

I understand I am responsible to ensure accuracy of the information listed on the client information forms including proper last names, especially in the case of weddings, names matching passports, dates of birth, etc are correct.

I am responsible for having up-to-date passports/travel documents/visas etc. as is required for travel and I am responsible for any consequences that may arise for not having them. I acknowledge that some countries have strict requirements for entry and that having a prior DUI or felony conviction of any kind could result in me being denied entry into that country or boarding in the case of a cruise vacation that docks in that country. Passports are required to have at least 6 months of validity remaining following your travel dates for many international destinations and airlines.

I understand that I need to check in at least 2 hours before my flight. Late check in could result in denial of boarding at my own expense.

I understand that I am ultimately responsible for keeping track of all payment due dates. Late final payments will result in forfeit of deposit.

I understand that Debit Cards often impose “Daily Charge Limits” and multiple payments may be needed to complete the full balance due; for which I will be notified if the situation occurs.

Life's a Journey Vacations will not release personal information to ANY parties not listed below, unless provided in writing as an Emergency Contact and will apply due diligence in maintaining records in a secured, safe location.

Be aware that there may be some entry and exit restrictions based on each country for visitors. These restrictions are set forth by the foreign governments and include DUI's, health concerns & vaccinations, child safety and support, offensive actions or any felony arrest history. It is every traveler's full responsibility to check and verify entry, exit, passport and visa requirements.
