I authorize KVR Travel Group LLC to charge the credit card indicated in this authorization form according to the terms outlined below. This payment authorization is for the goods/services described.
Unless specified otherwise, all payments are Non-Refundable.

I certify that I am an authorized user of this credit card and that I will not dispute the payment with my credit card company; so long as the transaction corresponds to the terms indicated in this form.

I also reviewed all trip details and confirm that all information is correct and any alterations to the trip may result in extra fees.

I acknowledge that I am purchasing this trip using a travel agent and not with the airline/hotel directly therefore the transaction cannot be disputed due to flight/hotel service dissatisfaction.

I am also responsible for having any up to date passport/travel documents/visas etc. , and I am responsible for any consequences that may arise for not having them.

I understand that I am responsible to keep track of all payment due dates. Late final payments will result in forfeit of deposit or of a loss of this vacation.

In the event of a cancellation, for any reason, KVR Travel Group LLC reserves the right, to charge this card a $150 Cancel Fee for time and services provided.
