By signing above, I acknowledge I am a legal parent or guardian of the above mentioned rider and I have read, understood and agree with all of the information presented above and below.

I, the parent or guardian of above named child, do hereby affirm that my child is fit to race and ride and do consent to my child’s participation. I have read and understood the agreement and release of liability. IN consideration of allowing my child to participate, I consent to it and agree that its terms shall likewise bind me, my child, my heirs, legal representative and assignees. I hereby release and shall defend, indemnify and hold harmless all the released parties from every claim and liability that I or my child may allege against the released parties (including reasonable attorney’s fees or costs) as a direct or indirect result of injury to me or my child due to my child’s participation in the race or ride, to the weather, or to the neglect of the released parties or others. I promise not to sue ERIK’S Bike Shop, Inc., ERIK’S Riders Club, or any released party on my behalf or on behalf of my child regarding any claim arising from my child’s participation.
