CAPTAIN REPORT
The information you submit here will be read by the department heads from Kitchen, Planning, Staffing, Warehouse. Sales reps/venue managers may also review reports about events at their own venues.
  • Starting from the left, select your date, location, and event name.
  • SET-UP

  • Note any incorrect or inconsistent information in the planning paperwork (timeline, service notes, floor plan, equipment review, etc.)
  • If bride or groom's name is incorrect, please note what the name was on the menu versus what the name was supposed to be. Example: The bride's name was Emily but the menu said Emma
  • Note factors that contributed to lack of set up time (e.g. staff late, truck late, not enough time allocated, client assigned extra duties, tent collapsed, venue destroyed by alien invasion, etc.)
  • EQUIPMENT

  • For example: "List said 5 racks of wine glasses, only 3 on truck" or "List said shooter glasses, we got votive glasses instead"
  • Describe what you needed, why it was needed, and in what quantity.
  • Definitely attach a photo using the link below.
  • This is useful for non-JAM sites where we may need extra carts, cords, etc.
  • THE STAFF

  • For example: Cyndi was 20 minutes late, Kevan had on purple socks and culottes
  • Phone useage and earbud usage on the job is officially prohibited by JAM policy. These are permitted only on breaks, or for valid operational reasons like calling for supplies, communicating with coworkers, consulting checklists, etc.
  • List names of top performers and what they did well
  • List names of low performers and what they need to work on
  • Applies only to events with 5 or more staff, not including captain. If not applicable, write "N/A".
  • THE FOOD

  • Attach a photo of the problematic food, please.
  • "Issues" means unanticipated food allergies, or where we were unprepared to deal with client food allergies.
  • Please be specific about food allergy issues.
    Examples:
    "A guest had a gluten allergy not listed on our paperwork" OR "A dairy free guest was listed on paperwork, but we did not have a dairy free meal for them."
  • IN CLOSING

  • Use this space to write anything about the party you wish to convey to management.
