Before completing this form, visit www.scoresports.com and review the equipment section of Score's website. From there, make notes about what equipment you will request, including quantity. You will be asked to submit this information with your request.

Iowa Soccer will review all requests received by the deadline, make donation determinations and update all organizations with the status of their requests. During the review process if there are follow-up questions, the person submitting will be contacted. Please note that submitting this request does not guarantee a donation, nor one that meets 100% of what has been requested.
