This form must be submitted annually to verify your team's eligibility to issue Student Service (SSL) hours for volunteer activities.

Proof of liability insurance must be submitted with this form as well as a description of what the students will be doing to obtain the SSL hours.

You will receive an email upon receipt. Please email ssl@mcsl.org if you do not receive a response within 48 hours.
