All Phi Alpha members nationwide are required to complete three (3) hours of volunteer work per quarter or semester. Community service is an essential aspect of Phi Alpha membership, and embodies the Phi Alpha intention to promote humanitarian goals.

This form is to be filled out and submitted quarterly to Phi Alpha – Tau Sigma. The form can be scanned and e-mailed to ONLY phialpha@mail.waldenu.edu. If you choose to volunteer at more than one agency, please use the space provided and/or an additional form to report your hours.
