I hereby grant permission for my youth to participate in Genesis’ Prep for Success workshop series.  I agree to exempt and relieve Genesis and its officers, directors, employees, or volunteers from liability for any personal injury which may be suffered by my youth, and I hereby voluntarily release, waive, and relinquish any and all loss of damages or actions or causes of action for personal injury, property damage or wrongful death occurring to my youth as a result of participation in this program.  I consent to any medical treatment or hospital care that may be deemed necessary for my youth in case of injury or medical need, and I authorize the director or sponsors in charge to initiate medical treatment in case of emergency.  As a parent or legal guardian of my youth, I am responsible for the health care decisions of my youth and am authorized to consent to the services to be rendered.  I represent that my consent to agree to pay for any medical or hospital care to be rendered to my youth is legally sufficient and that no consent from any other person is required.  I understand that Genesis emphasizes proper conduct and behavior by all participants while attending or participating in this activity.  I agree to support and encourage all these policies.  I hereby grant permission to Genesis to share information contained with the Program Director and facilitators as necessary.  The undersigned parent or guardian represents that he/she has read this release and understands and assumes all risks associated with this event.


I further hereby grant permission for photos taken during this event to be used strictly and only for purposes associated with Genesis promotions and information.  Any other usage is strictly forbidden.  Under those conditions I grant to Genesis, its representatives, and employees the right to take photographs of me and the above participants, including minors, during this event.


I authorize Genesis, its assigns, and transferees to copyright, use and publish the same in print and/or electronically. I agree that Genesis may use such photographs without my name, for any lawful purpose, including such purposes as publicity, illustration, advertising, and Web content.
