Please read this form carefully and be aware that in participating, either yourself or your child/ward, for any rental, camp, clinic, or any other soccer program affiliated with GoooalSports LLC or Soccer Academy CT, you will be waiving and releasing any and all claims for any injuries of any kind. This includes injuries of any kind sustained prior to, during or after play.
I, the participant/parent/guardian release and discharge Goooal Sports LLC, Soccer Academy CT, and any other person including, but not limited to employees, coaches managers, officers, owners, directors, officials, agents, or providers of transportation for children. This includes persons using their own vehicle, the heirs, administrators, executors, and successors, and hold them harmless against all claims, actions, causes of action, suits, damages, and liability of nature whatsoever arising out of the players participation in any Goooal Sports LLC, or Soccer Academy CT activity.
As a participant, parent/guardian of a participant in any program/activity, I recognize that there are certain risks of physical injury and I assume the full risk of any injury including, but not limited to, dehydration, concussions, sprains, trips, falls, breaks, disability, or death, any damages, or loss of any kind(personal or material) which I, or my minor child may sustain as a result of participation in any and all activities associated with Goooal Sports LLC, or Soccer Academy CT.
Not only do I agree to waive all claims, I hereby forever release and waive my right to bring suit against Goooal Sports LLC or Soccer Academy CT, but I further agree that the participant has been examined by a physician and is deemed fit and able to participate.
WAIVER OF LIABILITY RELATING TO COVID-19/CORONAVIRUS
The novel coronavirus, COVID-19, has been declared a worldwide pandemic by the World Health Organization. COVID-19 is reported to be extremely contagious. The state of medical knowledge and science relating to COVID-19 is very fluid and is evolving, but the virus is believed to be spread from person-to-person contact and/or by contact with contaminated surfaces and objects, and even possibly through the air. People reportedly can be infected and show no symptoms (asymptomatic) or can be contagious before symptoms appear therefore spread the disease to others. The exact method of spread and contraction are unknown, and there is no known treatment, cure, vaccine, or prophylactic medication for COVID-19. Evidence has shown the COVID-19 can cause very serious and potentially life-threatening illness and even death.
Goooal Sports LLC/Soccer Academy CT CAN NOT prevent you, or your child(ren) from being exposed to, contracting, or spreading COVID-19 while utilizing Goooal Sports LLC services or premises. It is not possible to prevent against the presence of the disease. Therefore, if you choose to utilize our services (of any kind) and or enter onto the premises of Goooal Sports LLC, you may be exposing yourself [or your child(ren)] to and/or increasing your risk of contracting or spreading COVID-19.
ASSUMPTION OF RISK: I have read and understood the above warning concerning COVID-19. I hereby choose to accept the risk of contracting COVID-19 for myself and/or my child(ren) in order to utilize/participate any and all Goooal Sports LLC and Soccer Academy CT services and enter the premises. These services or participation in any programs, activity, or viewing are of such value to me and/or to my child(ren), that I accept the risk of being exposed to, contracting, and/or spreading COVID-19 in order to utilize Goooal Sports LLC’s or Soccer Academy CT's services and the premises in person.
WAIVER OF LAWSUIT/LIABILITY: I hereby forever release and waive my right to bring suit against Goooal Sports LLC,Soccer Ac and its owners, officers, directors,ad managers, officials, agents, employees, or any other representatives in connection with exposure, infection, and/or spread of COVID-19 relating to utilizing Goooal Sports LLC premises. I understand that this waiver means I give up my right to bring any claims including personal injuries, death, disease, property losses, or any other loss, including but not limited to the claims of negligence and give up and claim I may have to seek damages, whether known or unknown, foreseen or unforeseen.
I HAVE CAREFULLY READ AND FULLY AND COMPLETELY UNDERSTAND ALL PROVISIONS OF THIS RELEASE AND FREELY AND KNOWINGLY ASSUME THE RISK FOR MYSELF AND/OR MY CHILD(REN) AND WAIVE MY RIGHTS CONCERNING LIABILITY DESCRIBED ABOVE:
