We agree to the following:

• We are here to promote a healthy relationship between you and your pet.
• We encourage you to follow up with us regarding any questions or concerns and ask that you respond to our follow up requests by email at: allpawsondeckrescue@gmail.com
• We recognize that pet matches may not be successful through no fault of the person or the pet. If your adoption does not work out, we do require that you return the pet so we may better assist matching the pet with a new home.
• We strive to provide you with a healthy pet. However, the stress of changing environments can lower an animal’s immunity to fight disease and the pet could harbor an infection without displaying symptoms. We cannot guarantee the health of any animal.

The Adopter & Co-Adopter agree to the following:

I am 21 years of age or older *

I realize that this animal may need further training or treatment. *

I may return the animal for an adoption refund within 72 hours for a previously undiagnosed health reason verified by a licensed veterinarian. We will not reimburse for medical bills. *

I will provide proper care, a human environment, and regular exercise and companionship for my pet. I will have the animal inoculated against rabies and abide by animal control laws. *Proper care includes routine immunizations, flea and heartworm prevention, veterinary care, adequate food, water, and shelter at all times. *

If adopting a cat, I will make sure that the cat is an INDOOR only cat. I understand that if the cat I am adopting is found to be outside the rescue has the right to refuse to return said cat back to me due to safety reasons and violation of this contract. *

If I am unable to provide proper care for the animal or can no longer care for the animal, I agree to return it to the rescue where I adopted from. *

I will not trade, sell, or abandon the animal. *

I understand that I will be given a copy of my pet’s kennel card which will have proof of all vaccines and medical history during the duration of their stay. *

I understand and agree that the animal has no express or implied warranty, representation or promise to the age, health, breed, habits, disposition, or safety of the animal. I hereby accept the animal as is, assume all risks and responsibilities associated with the ownership of the Animal, including bites. *

I understand that the Foster/Rescue is not responsible for any care of my pet once adoption is completed. *

I understand that the rescue does not test for Heartworms on any dog under 6 months of age. I understand the rescue does test for Heartworms on dogs over 6 months of age before adoption, but this does not guarantee that the dog may have had a false negative test, and the rescue will not be held responsible in this instance. *

I understand the rescue does test for FELV/FIV on all cats and kittens before adoption but this does not guarantee that the cat or kitten may have had a false negative test and the rescue will not be held responsible in this instance. *

I understand the rescue does test for internal parasites before adoption, but this does not guarantee that my pet may have had a false negative test, and the rescue will not be held responsible in this instance. *

I understand that my new pet may have been spayed or neutered before all his or her yearly vaccines were completed thus leaving the rest of the vaccines as my responsibility to continue through my regular veterinarian. I understand that my new pet does have age-appropriate vaccinations that are current up until this point but that does not mean they may not need more after adoption so please take all records to your regular veterinarian to find out if they may need further vaccinations. *

I understand that it is ILLEGAL to declaw a cat in Volusia County and by signing this contract I am agreeing that I WILL NOT declaw the animal I am adopting. *

Furthermore, I understand that outside medical expenses WILL NOT be reimbursed. *

I understand that the rescue WILL NOT reimburse my adoption fee if I need to return said animal unless it is for an undiagnosed medical issue diagnosed by a licensed veterinarian within 72 hours of adoption. *

NOTICE TO ADOPTORS

We recommend you keeping your new pet away from your existing pets for a 10-day quarantine period. You will need to be prepared for any additional veterinarian expenses needed for the pet that you are adopting. Any additional fees will be your responsibility. Please have your new pet checked by a veterinarian as soon as possible after adoption.
Thank you for saving a life by adopting a pet for your family. Please remember that adopting an animal is a SERIOUS commitment. Animals may take several weeks to months to settle into their new environment.
