Camp Button Parent Information form 2025
You are being asked to provide this information since you informed us your child has a special need. To provide the most enjoyable, safe and successful summer for your child, we need to know about your child. The information that you and teachers provide helps us determine the setting and modifications which are most appropriate for your child. Children accommodations are requested for will require a team meeting to determine appropriate camp placement.
Forms are due by May 23, 2025.

Camp Button (1/2 day) 11:45-3PM

Camp Button is a specialized camp for children ages 6-14 who require a more individualized camp experience. A typical Camp Button child is in a school program that is specific to their learning, behavioral and social needs based on their Individual Education Plan (IEP). The profile of a Button camper is a child who may have an educational classification of intellectual disability, autism, multiple disabilities or other health impairment. They are often in specialized programs such as ABA or self-contained settings, and require smaller settings or higher staff to child ratios. Camp Button provides activities, experiences and field trips that are accessible to each camper based on their level of cognition, adaptive abilities, sensory needs and social skills. Campers will have opportunities to engage in whole group games, small group activities and are provided individualized assistance from camp staff based on their level of need. Accommodations and modifications recommended by parents and the school team are considered and implemented based on accessibility at camp.

    Unless otherwise indicated, I grant permission to Groton Parks and Recreation to contact the school, teachers assistant, teacher, social workers, therapist or physician for the purpose of gathering or releasing information regarding this participant. The information will be used to provide the most effective plan for providing recreation services and proper placement in inclusion. All information will be kept confidential.
