The following information should be completed by the member seeking state support for running or bidding for a US Women of Today position. This form must be completed prior to Winter State convention. If you are planning to run for National President, you must indicate that on this form.

Responses to the online form will be viewed by the state chairman of the board, who is also the chairman of the Elections Committee. Fill in as much information as you know.
