Before proceeding, please read through this form carefully, as it is a legally binding agreement.

Next, we will gather additional information from all travelers, collect payment for the deposit, and finalize your decision on travel insurance.

Each traveler responsible for their own deposit must complete this form to secure their reservation. All roommates must book on the same day. Please ensure that everyone in your travel party submits their form as well. Reservations will only be confirmed once all roommate forms have been received. For group bookings, you and your roommate may proceed as soon as you're ready; there is no need to wait for other group members to book.

Please note that submitting this form does not automatically apply your payment; we will process payments from Emily’s Travel. Likewise, any credit card authorization forms you complete do not automatically charge your card.
