ALD Membership Order Form
Place your order for Alpha Lambda Delta membership certificates and honorary member certificates here. This form is for use by chapter advisors or officers only, on behalf of a campus chapter. It is not to be used by individual students to request a membership certificate.

To ensure timely delivery, please submit your order at least TWO WEEKS PRIOR to your initiation date.
  • Input the name of your institution exactly as it should appear on your membership certificates.

  • Please input a valid shipping address where the certificates should be sent. Please do not submit an off-campus address; ALD will not ship certificates to an off-campus address.

    Please input your list of new members and their email addresses into the space below, or use the "Upload file" button to upload a text file, Word document, or spreadsheet containing your new student members' names & email addresses.

    Please make sure to format names in STANDARD FORMAT - John Doe; Jane Roe; Mary Jones. Do NOT use Last/First format: Doe, John; Roe, Jane; Jones, Mary. Do NOT use all caps - capitalize the names as they should appear on the certificates.
  • Input names in standard format (First name followed by last name, no punctuation). Please put each name on a different line. Add EMAIL ADDRESSES in a separate column if you are allowed to share them.
  • Upload a text file, Word document, or spreadsheet containing your new members' names. Add EMAIL ADDRESSES in a separate column if you are allowed to share them.
  • Upload a text file, Word document, or spreadsheet containing your honorary members' names exactly as it should appear on the certificate. You may include pre- or post-nominal titles like Dr. or Ph.D., etc. Also include an email address for each honorary name.
