I understand that Kids’N Dance will provide instructor, tables and seating. Families may bring cake or cupcakes, and non-alcoholic beverages of their choosing. Families may arrive no more than 10 minutes prior to their party time.

I understand that all beverages must be non-alcoholic.

I understand that my reservation is for a specific time period. This time period includes clean up and saying goodbye to guests. Late departures will be charged to my credit card on file at the rate of $50/1-15 mins late or $100-16-30 mins late.

I understand that I may upgrade my party package prior to the scheduled party date and that If I have more participants than I paid for I will be charged $35 for each additional participant.

I understand that a 50% non-refundable downpayment is required at the time of booking and the balance is due on the day of my party and is to be paid in cash directly to my teacher. If the balance isn't paid at the end of the party we will chard your credit card the balance plus a $50 late fee

I understand that if I choose to reschedule my party I must give at least 10 days notice or additional fees may apply.

I understand that prices are subject to change and do not include gratuity.

I understand that parents of participants must drop off. The birthday child's family is welcome to come in but all others must drop off.

In checking the box above: I agree that Kids 'N Dance 'N Theater Arts is in no way responsible for the safekeeping of my personal belongings while I, my child, or my guests attend my private party. I understand that dance/circus/theater parties may be physically strenuous and I voluntarily participate in them or have my child/guests participate in them with full knowledge that there is risk of personal injury, property loss. I agree that neither I, my heirs, assigns or legal representatives will sue or make any other claims of any kind whatsoever against or its members for any personal injury, property damage/loss.

I voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury to my child(ren) or myself (including, but not limited to, personal injury, disability, and death), illness, damage, loss, claim, liability, or expense, of any kind, that I or my child(ren) may experience or incur in connection with my child(ren)’s attendance at KND.

I understand that at this time we are only allowing party participants and the birthday child's family in the building.

Liability Form: Onstage - Kids 'N Dance 'N Theater Arts, LLC (KND) has put in place preventative measures as recommended by the CDC, California and Alameda County Health Departments to reduce the spread of COVID-19. These measures are itemized in our Site Specific Covid Protection Plan and represents the policies and procedures we implement to help prevent the spread of Covid-19 to those in our space.

However, KND cannot guarantee that you or your child(ren) will not become infected with COVID-19. Further, attending KND could increase your risk and your child(ren)’s risk of contracting COVID-19.

By signing this agreement, I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that my child(ren) and I may be exposed to or infected by COVID-19 by attending KND and that such exposure or infection may result in personal injury, illness, permanent disability, and death. I understand that the risk of becoming exposed to or infected by COVID-19 at KND may result from the actions, omissions, or negligence of myself and others, including, but not limited to, KND employees, program participants and their families.
