Permit Loss Report
Please complete and submit the following information to obtain a replacement Permit. Permit Loss Reports must be submitted as soon as a valid Permit is lost, stolen, or no longer legible. Upon submission of this form, please bring your LMU ID and any other applicable material to the Campus Police & Security Office, located in the front lobby of Tex Turner Arena, open 7:30 AM – 2:30 PM, to pick up a replacement Permit.
