As a condition of membership in the National Llewellin Gun Dog Club, also referred to as the NLDC, I do hereby waive my right and the right of my heirs and assigns to hold the NLGDC, its directors, officers, or employees liable or responsible in any way for damage, injury, or death that may occur to myself, or my family or any of my properties or belongings, including animals. In addition, I agree to defend, indemnify and hold harmless the NLGDC and its directors, officers and employees against any and all loss and liability, damages and costs including attorneys fees arising from or relating to my participation, or my animals participation in which I have an ownership interest or which I am handling at any NLGDC event or activity. I understand that there are risks involved in the events and activities of the NLGDC, including without limitation animals allowed off leash from time to time and the use of and discharge of firearms. I agree to adhere to the rules, regulation, constitution and bylaws of the NLGDC. As a member of the NLGDC, I agree to conduct myself in a responsible manner at all NLGDC club meetings, events and activities; agree to use appropriate language in any public gathering. I agree to use the utmost caution and safe conduct while participating in or attending any NLGDC event or activity, and acknowledge that my membership privileges may be suspended or revoked for inappropriate behavior or misconduct prejudicial to the interests of the NLGDC.

Membership is good for one year. First year memberships include a window decal, voting privileges, club newsletters, and periodic updates on trial and club activities. By the submission of this form, I certify that I am not currently under suspension by the AFTCA, any Kennel Club, or Stud Book.
